Fees & Payment

Payment Options

Lighthouse Counseling Services provides professional counseling services to individuals, couples and families, and payment is required for services. Reduced rates are available but are limited to clients who qualify by application. See below.

“Private Pay Clients” pay for sessions entirely out-of-pocket in accordance with the Fee Schedule set forth in the Contract for Professional Services. Lighthouse Counseling Services accepts cash, checks, and credit cards. A typical weekly session costs $140 for 55 minutes. When cash is paid, clients will be provided a receipt to document the cash transaction. A more detailed

invoice can be provided upon request. 

“Insurance Clients” seek payment through their insurance company. Lighthouse Counseling Services is in-network with some companies, and out-of-network with others.

“Reduced Fees” are offered to clients who have extenuating financial circumstances. Clients who wish to apply for a reduced fee need to complete the Reduced Fee Application and submit it before scheduling their first appointment. Standard session rates will apply until the Application is processed. Applications can be faxed to (630) 325-4663, although original documents will need to be brought in and verified. Clients who wish to pay “reduced fees” cannot bill through insurance, as — according to our contract with insurance companies — we are not allowed to waive copays and deductibles for insurance clients. Insurance Options

Choosing to bill for counseling sessions through your insurance carrier is an important decision you must make. According to federal regulations, you may choose to pay out- of-pocket and NOT bill through your insurance policy. Clients who choose this option are called, “Private Pay Clients.” Should this be your preference, we (Lighthouse Counseling Services LLC) would NOT have the authorization to share your records with your insurance company. The decision you make at the outset of services may be reversed at any time by completing a new form and updating your file.

Lighthouse Counseling Services is “in-network” with the following providers:

• Blue Cross Blue Shield

• AETNA

• MultiPlan

We will bill most other insurance companies as an “out of network” provider, or we will accept payment at the time of services and provide you with an “insurance ready receipt” which you can use to get reimbursed directly from your insurance company.

Use this form to express your preference to be a “Private Pay Client” or an “Insurance Client.” You can use page two of the Insurance Verification Form to communicate with your insurance company and determine your benefits & out-of-pocket expenses BEFORE you make that decision!
